Why are there two Submit buttons in my Survey or Mailing?

Surveys and Mailings
Resolution:
Web forms must contain a Submit button. Two submit buttons will appear in a Survey or Mailing if there are two web forms. There will be a Submit button for every web form there is in the Mailing or the Survey.
Please ensure that you only have one web form per Mailing or Survey.
