Resolution:
You can create both text, choice and matrix type questions.
Text questions allow survey recipients to type an answer to the question in their own words. Text questions are open-ended questions, such as "How can we serve you better?"
When configuring a text question, you define the parameters of the response area, including the maximum number of characters allowed in the response as well as the size of the response box that displays (that is, how many lines and columns display for the response area).
Choice questions provide a list of options from which the recipient can select a single value or in some cases can select multiple values. When configuring a choice questions, the options must display in one of the following formats:
Matrix questions are used in cases where a series of questions can be responded to with the same set of answer choices. Matrix questions can help streamline a survey's appearance by grouping the questions together in a table. Answers are displayed using either radio buttons (to permit only one answer to each question) of check boxes (to allow more than one answer). If you choose radio buttons, you can restrict selections to one choice per row, forcing respondents to rank their answers.
