'Error Processing Document' error in notifications for surveys

Why am I seeing an 'Error Processing Document' error for surveys on my test/upgrade site?

Customers may see repeated error messages in their notification that read, 'site: Survey Name-Error Processing document.' on their newly created test or upgrade site. This error message is triggered when surveys are launched on the test/upgrade site, but no Outreach mailbox has been enabled.

In order to resolve the error message, the customer must either enable an Outreach mailbox on the affected interface or change the survey status to suspended.  See Answer ID 331: Setting up mailboxes in Oracle B2C Service for more information on mailboxes.
